Yes, you need to register each of your NAICs separately. Register by clicking on the New User Registration link on the Arizona IVS website.

What if we have a small Book of Business and filing FTP will put a strain on our technical resources?
If you insure less than 500 vehicles in Arizona an alternative reporting process is available to you. Please review Section 5, page 13 “BOB Reporting by Smaller Insurance Companies” of the Implementation Guide for additional Details.

How can we correct/change the contact information for our company?
Log in to the www.ArizonaIVS.com website, click on Account Information, and change the contact information.

I have forgotten my User Name, how can I retrieve it?
For the main insurance company user, your User Name is the same as your NAIC number. For insurance company sub-users, your user name is your email address.
Q.
I have forgotten my password. How do I reset it?
You can reset your password by selecting the Forgot/Reset Password link located on the log in screen of the AZIVS website. After selecting this link you must provide the answer to the secret question. A temporary password will be sent to the main/functional contact for the account by answering the question correctly.

Is the website password totally independent of the FTP account credentials?
Yes, they are independent.
Q.
We represent three NAIC codes. Can we report all three companies in one Book of Business file or should we send three separate files?
You must submit one Book of Business file per NAIC number.

How do I change my web account password?
Login to the www.ArizonaIVS.com website using your username and password, Select "Account Information" in the top menu, Check the "Do you want to update your password?" check box and follow the steps. If you have previously submitted a secret question and answer, then the answer will be needed to modify the password.

Could you provide further clarification on the vehicle types that are exempt from reporting to Arizona IVS?
Arizona Revised Statue Title 28, Chapter 9, Section 4132 lists the vehicles that are exempt from the mandatory insurance requirements. A copy of the statute may be found at:

The vehicle types specified in the law are listed below.
1. Farm tractor.
2. Trailer used solely in the operation of a farm for transporting the unprocessed fiber or forage products of a farm or an implement of husbandry designed primarily for or used in agricultural operations and only incidentally operated or moved on a highway.
3. Road-roller or road machinery, including a power sweeper, temporarily operating or moved on the highway.
4. Trailer not used for commercial purposes or semitrailer not used for commercial purposes.
5. Motor vehicle rented without a driver that meets the requirements of section 28-2166.
6. Motor vehicle registered pursuant to section 28-2154.
7. Motor vehicle owned by the United States government.
8. Golf cart used in the operation of a golf course or only incidentally operated or moved on a highway.
9. All-terrain vehicle or off-road recreational motor vehicle operating on a dirt road that is located in an unincorporated area of this state. For the purposes of this paragraph, "dirt road" means an unpaved or ungraveled road that is not maintained by this state or a city, town or county of this state.
10. Off-highway vehicle participating in an off-highway vehicle special event as defined in section 28-1171.

Is Commercial reporting required?
Yes, commercial reporting is required. Please refer to the Implementation Guide for details regarding commercial reporting.
Q.
Should vehicles on comprehensive only polices be reported?
No, you should only include vehicles with active liability insurance.

Will SR-22/26 filings be sent in the same file with the BOB?
No, BOB is required to be reported weekly (once every 7 days) and SR22/26 should be submitted daily in SR22/26 files that have distinct format from the BOB files. For additional details on SR22/26 reporting, please review Section 6 of the Implementation Guide.

When can we stop reporting to AMIRS (Arizona Mandatory Insurance Reporting System)?
The legacy AZ insurance reporting system AMIRS (Arizona Mandatory Insurance Reporting System) has been discontinued. Insurance companies can stop reporting to AMIRS.

Can foreign/domestic surplus lines carriers report to Arizona IVS?
Yes, foreign and domestic surplus lines carriers are required to report to AZ IVS

What if a vehicle owner received an ADOT DMV insurance suspension notice and has insurance?
If the vehicle owner has insurance, they will need to contact their insurance company or agent. The insurance company or agent will then need to electronically transmit the insurance information using the ArizonaIVS website. The insurance company/agent will need the reference number included in the notice or vehicle VIN in order to submit the insurance information.

What if a vehicle owner received an ADOT DMV insurance suspension notice and the vehicle is stored or inoperable?
If the vehicle is not being driven on public roads, the customer can complete a De-Insured Certificate application at AZMVDNow.gov

What if a vehicle owner received an ADOT DMV insurance suspension notice and the customer’s vehicle has been sold?
If the customer no longer owns the vehicle, the customer can complete a sold notice at AZMVDNow.gov.
Q.
What happens if the vehicle owner does not respond to the ADOT DMV insurance suspension notice?
If the vehicle owner fails to comply with this notice by the date specified in the notice (15 days after incident date), the registration will be suspended and a $50 reinstatement fee will be required.

Where can the customer pay the reinstatement fee.
The customer can pay the reinstatement fee at AZMVDNow.gov
Q.
The insurance company/agent submitted insurance for the vehicle. However, the registration is still suspended.
The insurance policy effective begin date has to be prior to the prohibition (suspension) effective begin date.
To clear the suspension:
- The customer has to pay the $50 reinstatement fee at AZDMVNow.gov
OR
- Insurance that began before the suspension effective date has to be submitted by an insurance company or agent.
